Athletic Application

All athletes are required to fill out the online registration through FamilyID to participate in any sports.  All athletes must have a physical completed and form signed by a person licensed to practice medicine or osteopathy. Download and print the Physical History & Exam form below:

Fees

Students that participate in athletics at Cathedral are required to pay an Athletic Fee. These fees are billed upon receipt of the roster from the coaches of the various sports. The fees go towards helping to offset the costs of the sports including coaches, field and court maintenance, jamboree and tournament entry fees, officials, and safety equipment. Athletic Fees must be up to date and paid in full for the current sport team before a student can tryout and participate in the next season’s sport. Fees must also be paid before the next Report Card will be released. The cost for each participant is by sport.

All others: $100
